I just didn't know it was that cuz my hard drives light was always on. I got the almost same problem, my ps4 always tell me the external USB drive is disconnected, I need to reconnect the USB again to fix this. On the other hand, USB 3.2 or USB-C ports are faster than USB 2.0 ports.

I'm using a 3.0 HD and cable, but everytime I turn on my ps4 I get an error code stating it needs to be repaired. Minimum of 250 GB and a maximum of 8 TB capacity.

You will get an error message, if the same application is installed on the system storage and the external storage device. Both of these things could cause your problem.
